If you would like to fill out your updated information form ahead of time, simply print and fill out the attached form and bring it in with you to your examination appointment. Even if you've filled out this form in the past, we'll need an updated form for you at the time of each annual exam.

ATTENTION VSP MEMBERS: In addition to the Patient Information Form, you must complete and sign this VSP Consent Form for our records at the time of each annual exam. This form is required specifically by your insurance company.
